Form I - Is there documentation in the record that the participant ever had… 1. Hypertension or high blood pressure? 2. Congestive heart failure? 3. A myocardial infarction or heart attack? 4. Other heart conditions such as problems with heart valves or the rhythm of your heartbeat? 5. A stroke? 6. Emphysema or asthma or chronic obstructive pulmonary disease (COPD)? 7. Diabetes, high blood sugar, or sugar in the urine? 8. Any cancer? 9. Liver disease (such as hepatitis)?

Emphysema or asthma or COPD - Emphysema is a type of chronic lung disease that causes difficulty breathing and is commonly associated with smoking. Asthma is a condition in which airways narrow and swell and produce extra mucus which triggers coughing, wheezing, and shortness of breath. Chronic obstructive pulmonary disease (COPD) is an inflammatory lung disease that causes obstructed airflow from the lungs leading to breathing difficulty, coughing, and wheezing

Details

This measure is to be collected from the medical record for all participants. Review of discharge summaries, history and physicals, and rehabilitation physician progress notes is adequate. Any new condition that occurs during the acute or rehab stay should be counted, along with conditions listed in the patient’s prior health history.

Unknown code is to be used when no records are available or records state “unknown medical history”.

If a condition is documented in one medical record source but not another, then keep. For example, if mentioned in the acute record, but not in the rehab record it should be kept. Vice versa, if mentioned in the rehab record but not in the acute record it should be kept.

Do not include condition if only mentioned by interview.

Code CAD under “other heart conditions” unless chart or person mentions myocardial infarction (MI), heart attack, or congestive heart failure (CHF). If MI, heart attack, or CHF is also noted then use those categories.

Low blood pressure is only a problem if symptomatic. If not symptomatic it should not be counted in any of the health conditions.

Only long-lasting conditions should be included, so incidences such as “hypertensive on admission to the ER” would not be included.

Do not include condition if only found as an ICD code.

For items 1-8: Medicare survey questions #20, 22-26, 32, 33 Medicare Survey: SAMPLING METHODOLOGY 2009 Cohort 12 Baseline Sampling CMS identified beneficiaries who were eligible for sampling as follows: * MAOs with fewer than 500 members were not required to report HOS. * For MAOs with 500 to 1,200 members, all eligible members were included in the sample. * For MAOs with more than 1,200 members and less than 3,000 members, a simple random sample of 1,200 members was selected for the baseline survey. * For MAOs with 3,000 or more members, members who responded to the 2008 Cohort 11 Baseline survey were excluded from the 2009 Cohort 12 Baseline sample. * Members were defined as eligible if they did not have End Stage Renal Disease (ESRD). The six months enrollment requirement was waived beginning in 2009.

National Health and Nutrition Examination Survey (NHANES) The NHANES interview includes demographic, socioeconomic, dietary, and health-related questions. The examination component consists of medical, dental, and physiological measurements, as well as laboratory tests administered by highly trained medical personnel. Findings from this survey will be used to determine the prevalence of major diseases and risk factors for diseases. Information will be used to assess nutritional status and its association with health promotion and disease prevention. NHANES findings are also the basis for national standards for such measurements as height, weight, and blood pressure. Data from this survey will be used in epidemiological studies and health sciences research, which help develop sound public health policy, direct and design health programs and services, and expand the health knowledge for the Nation.

NHIS National Health Interview Survey (NHIS) The National Health Interview Survey (NHIS) has monitored the health of the nation since 1957. NHIS data on a broad range of health topics are collected through personal household interviews. For over 50 years, the U.S. Census Bureau has been the data collection agent for the National Health Interview Survey. Survey results have been instrumental in providing data to track health status, health care access, and progress toward achieving national health objectives.

NHANES (NATIONAL HEALTH AND NUTRITION EXAMINATION SURVEY - ARCHIVE

Definition

Types of conditions diagnosed, along with if the onset was before, after or about the same time as the TBI, and age that they were first told they have the condition. All definitions provided below are from Mayo Clinic (http://www.mayoclinic.org) except chronic pain. If a participant asks for a definition of the disease, it is acceptable to tell them the following:

Heart Arrhythmia: Occurs when the electrical impulses that coordinate heartbeats don’t work properly and cause the heart to beat too fast, too slow, or irregularly
Emphysema or asthma or COPD: a type of chronic lung disease that causes difficulty breathing and is commonly associated with smoking
Pneumonia: Lung infection caused by bacteria, viruses, or fungi
Sleep disorder like sleep apnea: A medical problem with drops in oxygen levels during sleep that prevents a person from sleeping normally; other examples are narcolepsy and restless legs syndrome Cataracts: A clouding of the normally clear lens of your eye. For people who have cataracts, seeing through cloudy lenses is a bit like looking through a frosty or fogged-up window.
Chronic Pain: Pain is actually a symptom, not a diagnosis. Pain is a self-report item. However, there are diagnosis in which chronic pain is a central feature. While acute pain is a normal sensation triggered in the nervous system to alert you to possible injury and the need to take care of yourself, chronic pain is different. Chronic pain persists. Pain signals keep firing in the nervous system for weeks, months, even years. There may have been an initial mishap – sprained back, serious infection, or there may be an ongoing cause of pain – arthritis, cancer, ear infection, but some people suffer chronic pain in the absence of any past injury or evidence of body damage. Many chronic pain conditions affect older adults. Common chronic pain complaints include headache, low back pain, cancer pain, arthritis pain, neurogenic pain (pain resulting from damage to the peripheral nerves or to the central nervous system itself), psychogenic pain (pain not due to past disease or injury or any visible sign of damage inside or outside the nervous system). A person may have two or more co-existing chronic pain conditions. Such conditions can include chronic fatigue syndrome, endometriosis, fibromyalgia, inflammatory bowel disease, interstitial cystitis, temporomandibular joint dysfunction, and vulvodynia. It is not known whether these disorders share a common cause
Alcoholism: Pattern of alcohol use that involves problems controlling your drinking, being preoccupied with alcohol, continuing to use alcohol even when it causes problems, having to drink more to get the same effect, or having withdrawal symptoms when you rapidly decrease or stop drinking
Drug addiction: disease that affects a person’s brain and behavior and leads to an inability to control the use of a legal or illegal drugs or medications.
Depression: Mood disorder with persistent feelings of sadness with or without loss of interest. This problem interferes with daily activities and cause significant distress
Anxiety: disorder with persistent, intense, and excessive worry and fear about everyday situations. This problem interferes with daily activities and cause significant distress
Bipolar disorder or manic depression: Mental health condition that causes extreme mood phases that include emotional highs (mania or hypomania) and lows (depression). This problem interferes with daily activities and cause significant distress
Attention deficit disorder (ADD)/Attention deficit hyperactivity disorder (ADHD): Includes a combination of persistent problems such as difficulty paying attention, hyperactivity, and impulsive behavior across settings. This problem interferes with daily activities and can cause significant distress
Obsessive-compulsive disorder: features a pattern of unwanted thoughts and fears (obsessions) that lead you to do repetitive behaviors (compulsions). These obsessions and compulsions interfere with daily activities and cause significant distress

Details

This measure can be collected from best source available during the Form 2 interview for all participants. Conditions with positive responses will remain positive and should not be asked again on subsequent follow-ups.

For conditions that are present, the follow-up questions should be asked:
- ‘Was that before, after or about the same time as your TBI (insert number of years since TBI)?’, followed by; ‘How old were you when a doctor or other health professional first told you that you had this condition?’

If the participant has difficulty remembering their age at time of diagnosis, they will be asked:
- ‘How many years ago did a doctor or other health professional first tell you that you had this condition?’

First administration: For participants being administered the NHANES for the first time since study enrollment ask “has a doctor or other health professional ever told you that you had…” for each medical condition. For each health condition endorsed with “yes” the data collector will follow-up with “at what age were you diagnosed?”

Follow-up administration: For participants who were previously administered the NHANES, and are now being administered a follow up NHANES ask, “In the past year has a doctor or other health professional told you that you have…” For any health condition endorsed with “Yes” follow up with additional questions (e.g., ‘At what age were you diagnosed?’).

Age: On questions asking for the age of diagnosis, obtain the age of diagnosis. It is permissible for participant to estimate if they have an idea for when they were diagnosed, but should not make up a random age if they truly have no idea. Prompt if the diagnosis occurred during childhood, adolescence, their 20s, their 30s, etc.

If a health condition was diagnosed prior to one year, enter “0”.

Before, after or about the same time as TBI: A 6 month window on either side of the injury date would be considered to be ‘about the same time’ as TBI.

If the participant has difficulty remembering their age at time of diagnosis, they will be asked:
* ‘How many years ago did a doctor or other health professional first tell you that you had this condition?’

Do not accept self-diagnosis or a diagnosis that does not come from a doctor or other health professional. “Doctor” is meant to include health care providers who diagnose medical conditions.

The following are acceptable: - Medical Doctors (MD) in all medical specialties including Psychiatrists - Doctors of Osteopathic Medicine (DO) - Physician Assistants (PA) -Nurse Practitioners (NP) - Psychologists, Neuropsychologists (Ph.D. or Psy.D) - Podiatrists (DPM)

Not acceptable (these providers treat but do not diagnose) - Speech Pathologists (SLP) - Registered Nurses (RN) - Physical Therapists (PT) - Social Workers (LSW, LICSW) - Occupational Therapists (OT) - Naturopathic Doctors (ND) - Counselors (LMHC, LMFT, CRC) - Chiropractors (DC)
