GAD
Definition
The Generalized Anxiety Disorder 2-item (GAD-2) is a brief initial screening tool for generalized anxiety disorder.
The full Generalized Anxiety Disorder Scale is a 7-item scale validated as a screener for anxiety disorder.
Participants are asked how often they have been bothered by the first 2 items from the list below over the last 2 week. If either is endorsed, the remaining items are asked.
- Feeling nervous, anxious or on edge
- Not being able to stop or control worrying
- Worrying too much about different things
- Trouble relaxing
- Being so restless that it is hard to sit still
- Becoming easily annoyed or irritable
- Feeling afraid as if something awful might happen
- If you indicated any problems in the previous questions, how difficult have these problems made it for you to do your work, take care of things at home, or get along with other people

Details
Interviewers should read the following introduction prior to administering the GAD: “Over the LAST 2 WEEKS, how often have you been bothered by the following problems?”
If either of the first 2 questions are coded either ‘1 - Several Days’, ‘2 - More Than Half Of The Days’, or ‘3 - Nearly Every Day’, then proceed to ask the remaining GAD items.
If both of the first 2 questions are coded ‘0 - Not at all’, code remaining GAD items as ‘81 - Not Applicable’ and skip to next section of interview.
The GAD should not be administered to a significant other, or any other proxy. If the individual is unable to provide data, use code ‘82. Not Applicable: No data from person with TBI’.
Every effort should be made to obtain the GAD assessment, however, if any items can not be assessed, use code ‘99. Unknown’. Do not leave blanks.

GAD - CALCULATED
Definition
This calculated variable aggregates the scores of the seven individual GAD items to produce the total GAD-7 score, which is a screening tool for anxiety disorder. The logic handles missing data, initial screening conditions, and non-applicable statuses based on the source item responses.
Logic
Filter 1: Future Follow-up Date (Not Calculated) If the Followup date is in the future (meaning the assessment hasn’t been completed yet), the output is NULL.
Filter 2: Missing Data (Not Applicable: No TBI data) If the required initial screening question (GADNervousF) is coded as 82 (No data from person with TBI ), the calculated result is 888 (Not Applicable: No data from person with TBI).
Filter 3: No Problems Endorsed (Score of 0) If both initial screening questions (GADNervousF and GADCntrlWryF) are coded as 0 (Not at All ), the total score is 0. This reflects the screener logic where all subsequent items are coded 81 - Not Applicable: No problems.
Primary Calculation: Sum of Valid Responses (Score 0-21) If all seven GAD items (GADNervousF, GADCntrlWryF, GADWorryF, GADRelaxF, GADRestlessF, GADAnnoyF, GADAfraidF) have valid response codes (0 through 3 ), the final score is the sum of those seven individual item scores.
Default Fallback (Unknown) If none of the above conditions are met (e.g., if one or more essential items contain an 81 - Not Applicable from the skip logic, or a 99 - Unknown ), the calculated result is 999 (Unknown).
