DEPRESSION

Definition
The Patient Health Questionnaire-2 (PHQ-2) is a brief initial screening tool for depression.
The Patient Health Questionnaire-9 (PHQ-9) contains is a 9-item, patient self-report depression assessment.
Participants are asked how often they have been bothered by the first 2 items from the list below over the last 2 week. If either is endorsed, the remaining items are asked.
- Little Interest or pleasure in doing things
- Feeling down, depressed, or hopeless
- Trouble falling or staying asleep, or sleeping too much
- Feeling tired or having little energy
- Poor appetite or overeating
- Feeling bad about yourself – or that you are a failure or have let yourself or your family down
- Trouble concentrating on things, such as reading the newspaper or watching television
- Moving or speaking so slowly that other people could have noticed. Or the opposite - being so fidgety or restless that you have been moving around a lot more than usual
- Thoughts that you would be better off dead, or of hurting yourself in some way
- If you indicated any problems in the previous questions, how difficult have these problems made it for you to do your work, take care of things at home, or get along with other people

Details
Interviewers should read the following introduction prior to administering the PHQ: “Over the LAST 2 WEEKS, how often have you been bothered by the following problems?”
If either of the first 2 questions are coded either ‘1 - Several Days’, ‘2 - More Than Half Of The Days’, or ‘3 - Nearly Every Day’, then proceed to ask the remaining PHQ items.
If both of the first 2 questions are coded ‘0 - Not at all’, code remaining PHQ items as ‘81 - Not Applicable’ and skip to next section of interview.
The PHQ should not be administered to a significant other, or any other proxy. If the individual is unable to provide data, use code ‘82. Not Applicable: No data from person with TBI’.
Every effort should be made to obtain the PHQ assessments, however, if any items can not be assessed, use code ‘99. Unknown’. Do not leave blanks.

PHQ - CALCULATED
Definition
This calculated variable aggregates the scores of the nine individual PHQ items (which are a self-report depression assessment) to produce the total PHQ-9 score, ranging from 0 to 27. The calculation incorporates the screening and data availability rules applied to the PHQ items.
Logic
Filter 1: Future Follow-up Date (Not Calculated) If the Followup date is in the future (meaning the assessment has not been successfully completed), the output is NULL.
Filter 2: Missing Data (Not Applicable: No TBI data) If the primary screening question (PHQPleasureF) is coded as 82 (Not Applicable: No data from person with TBI) , the calculated result is 888 (Not Applicable: No data from person with TBI).
Filter 3: No Problems Endorsed (Score of 0) If the two initial screening questions (PHQPleasureF and PHQDownF) are both coded as 0 (Not at All), the total score is 0. This reflects the PHQ-2 screener logic where individuals endorsing “Not at all” for both of these items are generally scored as 0 and skip the remaining items.
Primary Calculation: Sum of Valid Responses (Score 0-27) If all nine PHQ items (PHQPleasureF, PHQDownF, PHQSleepF, PHQTiredF, PHQEatF, PHQBadF, PHQConcentrateF, PHQSlowF, PHQDeadF) have valid response codes (ranging from 0 to 3 for each), the final score is the sum of those nine individual item scores.
Default Fallback (Unknown) If none of the above conditions are met (e.g., if one or more essential items contain an 81 - Not Applicable from the skip logic, a 99 - Unknown , or any other invalid value), the calculated result is 999 (Unknown).
